Guidelines for filling out and submitting the User Request Form:

• Complete the User Request Form ensuring all the required fields have been completed.
• Please ensure all the required information is complete, concise, and accurate.
• Once the form has been completed please send the form to health.ehrvendorsupport@gov.ab.ca 
• Once the request has been processed an email will be sent to the requestor with the necessary access information.

* Please note this process generally takes 2-3 business days.
