Central Patient Attachment Registry (CPAR) is a provincial repository that will provide a dependable provincial source of attachment relationships between Albertans and their health care providers.

To manage CPAR access for staff members at your location (create, modify or delete), please complete and submit a user registration form.
To complete the form:
- Save a copy of either the Crowfoot Village Family Practice User Registration eForm or the Sylvan Family Health Centre Clinical ARP User Registration eForm to your computer to make sure you are working from the current version.
Custodians may act as their own Access Administrators, or they can delegate this responsibility to another staff member at their location. The following form will help you manage Access Administrator delegation for a staff member at your location. Ether:
